- Written application to the Zoning Officer with owner names and addresses, a description of the dwelling and any managing agent
- Fee paid in full when the application is filed
- Inspection finding the dwelling safe and fit for human habitation and in line with state statutes and Borough housing and health ordinances; violations must be corrected before the certificate is issued
- Combined form also covers the residential fire inspection: number of smoke detectors, CO detectors and fire extinguishers (form still lists extinguishers)

Required- Requirements
- Smoke alarms located and maintained per N.J.A.C. 5:70-4.19 and NFPA 72
- Carbon monoxide alarms per N.J.A.C. 5:70-4.19 and NFPA 720
- Portable fire extinguisher per N.J.A.C. 5:70-4.19(e) (Borough code and form still list it)
- Installed fire alarm systems may need testing and maintenance records before the certificate is issued
