- Requirements
- Occupancy Transfer Inspection of the home before the sale (the form covers a rental or a sale)
- Submit the Smoke and Carbon Monoxide Alarm Certificate Application together with it
- Give owner, purchaser, tenant and lock box details and the building type, bedrooms, heat, sewer/septic and water/well on the form

Smoke and CO alarm certificate
Required- Requirements
- 10-year sealed battery smoke alarm in every bedroom (depending on construction year)
- Basement smoke alarm at the bottom of the stairwell at ceiling level (not between joists)
- Hallway smoke alarms within 10 feet of all bedroom doors
- Electric smoke alarms required by the building code cannot be replaced with battery alarms
- Smoke detectors at least 3 feet from ceiling fan blade tips and air ducts; none in kitchens, bathrooms, dead air space, crawl spaces or attics
- CO alarm in the hallway within 10 feet of all bedrooms; not plugged into a switched outlet
- Fire extinguisher: residential 2A:10B:C, no larger than 10 lb, mounted within 10 feet of the kitchen (April 2026 form still lists it)
